  • 3-5 Year Goal 

    This is your future state — your North Star. 

“Strengthen our role as a regional anchor in health equity efforts.”


  • Annual Goal

     What progress do we want to see this year? 

    “Establish three formal partnerships and pilot one cross-sector initiative.”


  • Quarterly Focus (“Benchmarks”)

     What 2–4 key actions will move that annual goal forward — this quarter?

     “Host one partner strategy session, draft initiative concept, finalize MOU with the health system.”

Before jumping into task lists or timelines, align your team around 2–4 priorities for the quarter. 


Ask: What progress will matter most in the next 90 days?


Then — make those benchmarks visible. Use them in staff meetings, track progress weekly, and celebrate completion.
